In-home physical therapy · Miami-Dade County

Pain has a reason. We'll find yours.

Most people arrive with an imaging report and no explanation. Those are two different things. We bring a full movement evaluation to your living room — and leave you with an answer you can actually repeat to someone else.

Tell us where it hurts — we usually reply the same day.

Restore, move, and revive.

No clinic, no waiting room. The evaluation happens where you actually live and move.
The same two people every visit. No rotating staff, nothing lost between sessions.
You can start without a referral. Florida lets you come straight to a physical therapist.
If it isn't a movement problem, we'll say so — and point you to the right person.

Does this sound familiar?

You're not imagining it.
You just haven't been examined closely enough.

"The scan came back normal — but it still hurts."
"I've been handed the same sheet of exercises twice."
"The pain moved, and now nobody can tell me why."
"I stopped doing something I love and just built my life around it."
"Every appointment is fifteen minutes and a new person."
"I'm managing it. I'm not actually better."

Pain that hasn't resolved is usually pain that hasn't been fully explained. The examination is the part we care most about — because everything after it is only as good as the reasoning behind it.

The 60-second self-check

Try a piece of the exam right now

Pick where it bothers you and do three slow movements. Each one shows what a physical therapist would be watching for — and your answers become the message you send us, so we start already knowing something real about you.

Move slowly, and stop if anything is sharp. This is a way to notice things, not a diagnosis, and it is not medical advice. The point is simply to give us better information to work with — and to give you language for what you are feeling.

Seek medical care now, not a physical therapy visit, if you have: numbness in the groin or inner thighs, new loss of bladder or bowel control, unexplained weight loss or fever alongside the pain, sudden severe weakness in an arm or leg, or chest pain and shortness of breath.

What actually happens

Your first visit, start to finish

One appointment, in your home, with a Doctor of Physical Therapy. Here is exactly how it goes.

Typically 60–75 minutes
  1. 1

We listen to the whole story

When it started, what makes it worse, what you've already tried, and what you've stopped doing because of it. The history often points at the answer before we ever put hands on you.

  2. 2

We test — we don't guess

A structured movement examination: strength, joint mechanics, nerve mobility, balance, and how much load the area actually tolerates. Each test either rules something in or rules it out.

  3. 3

We screen for what isn't ours to treat

Part of a physical therapy evaluation is recognizing when something falls outside our scope. If that's what we find, you'll hear it that day, along with who you should see.

  4. 4

You get the explanation in plain language

What we found, what it means, and why it's been behaving the way it has. Clear enough that you could explain it to your spouse or your physician without notes.

  5. 5

You leave with a plan, and we start it

What we'll do together, roughly how long it should take, what you'll do between visits, and how we'll know it's working. Treatment begins in that same session.

Dr. Diaz's favorite part of the job is the part most people never get: the moment the pain finally makes sense.

Who comes to your door

A husband-and-wife practice

ProSouth is two licensed clinicians who work the same caseload together — which is why nothing gets lost in a handoff.

Evaluation & plan of care

Dr. Yelieseth Diaz

PT, DPT — Licensed Physical Therapist, State of Florida

Yelieseth leads every evaluation. Her strength is clinical reasoning — working through what a body is doing and why, especially for people who've been told nothing is wrong. She builds the plan and adjusts it as the picture sharpens.

Treatment & progression

Adrian Exposito

PTA — Licensed Physical Therapist Assistant, State of Florida

Adrian carries the plan through session to session — the hands-on work, the loading, the progressions, and the week-to-week accountability that decides whether a plan actually holds.

They're married. They talk about your case at dinner. That is either the most reassuring thing on this page or the strangest — but it's why the person treating you on Thursday already knows exactly what was found on Monday.

Getting started

Three steps, no paperwork first

01

Message us on WhatsApp

Tell us where it hurts and roughly how long it's been going on. A few sentences is plenty. We'll tell you honestly whether we can help.

02

We come to your home

We bring what we need. You need a bit of floor space and clothes you can move in — no equipment, no clinic, no parking garage.

03

You get answers, then work

The evaluation, the explanation, and the start of treatment all happen in that first visit. Then we keep going until you don't need us.

Questions people ask first

Straight answers

Do I need a referral from a doctor?

No. In Florida you can begin physical therapy without a physician's referral. If your care continues beyond 30 days for a condition a practitioner hasn't already evaluated, Florida law asks that a licensed practitioner — a physician, osteopathic physician, chiropractor, podiatrist, dentist, or an advanced practice registered nurse — review and sign your plan of care. We coordinate that for you; it isn't something you have to chase down.

What if it turns out not to be a physical therapy problem?

Then we tell you that day and point you toward the right provider. Screening for conditions outside our scope is a required part of a physical therapy evaluation, not an afterthought. You'd leave with a clear next step rather than another appointment.

Where do you travel?

All of Miami-Dade — from Homestead and Florida City in the south, up through Kendall, Coral Gables and Miami, and out to Key Biscayne, Miami Beach, Surfside, Bal Harbour, Sunny Isles and Aventura. If you're right on the county line, message us anyway; depending on the day we may still be able to come to you.

What do I need to have ready at home?

Comfortable clothes you can move in, a small area of open floor, and any imaging reports or notes you already have. That's genuinely it — we bring the rest.

¿Hablan español?

Sí. Toda la evaluación, la explicación y el plan de tratamiento se pueden hacer completamente en español. Escríbanos por WhatsApp en el idioma que prefiera.

Can you see me if I can't have someone come to the house?

In some cases, yes — we can start with a video visit to talk through your history and watch how you move, then decide together whether an in-home evaluation makes sense. Some problems genuinely need hands on them; we'll tell you which kind yours is.

How soon can you come?

It depends on the week, but we're a small practice, so scheduling doesn't go through a call center. Message us on WhatsApp and you'll usually hear back the same day.

Tell us where it hurts.

One message is enough to start. If we're not the right fit for what you're dealing with, we'll say so and tell you who is.
